Item disappeared from inventory after disconnect

I was at "moment of zen" map. Got Mind of the Council, put it in my inventory.

I was switching my old helm to see the stats changing. The moment I picked up the unique helm from inventory I got disconnected without able to put it in head slot or back in inventory.

Quick reconnect, "map failed" and unique has gone.
