Warrior/Titan missing most of its skills

The warrior seems to miss every second even first attacks I keep dying to mobs because I cant kill them ,it even happens on bosses I cant clear rooms and cant go on because I keep dying because of all the misses


PLS let me know if my build is wrong or if I am doing something wrong
