Can't start wave 15 simulacrum.

Reference # is: 3,910,957,962

Made it to wave 15 simulacrum but can't interact with the NPC for the final wave. The chest also did not spawn after completing wave 14.

Not sure if related but I'm using the Rite of Passage unique charm that spawns the apes. On wave 14, Kosis teleported out of the arena to the monkeys in the previous arena. I had to wait for the monkeys to slowly kill Kosis. It bugged out after that.

Edit: It also says 6 enemies remaining when there are no enemies in the immediate arena.

I have the exact same issue, but on wave 11. I also use Rite of Passage with the primate spirit. Happened twice in a row..

Just googled this problem and this is the first response. Thread is from 0.3, but I had the same issue in 0.4. Also have the Primate Spirit Rite of Passage, bugged after wave 13 for me. Please fix, very frustrating to happen that late in Simulacrum.
