Nexus "completed"

Right after fighting the boss of the Nexus I died before anything could happen, after respawning the map said it was cleared yet still left with the quest to complete that Nexus. (died before any npc could spawn and got no rewards)

I have the same issue

I also got the same bug twice and I still have zero Atlas points.
