Black Crow boss does not phase consistently

2nd time I've ran into this.

Fighting the Black Crow map boss and he does not go into his second phase. He becomes invulnerable to damage, does not transition to the second phase and continues to attack.

I get soft locked and cannot continue playing the game since I'm locked to the boss. Relogging doesn't help.

I could die to reset the encounter but I believe we can all agree that is not an ideal outcome.

This has happened to me just now as well. Very frustrating.
