Silver chalice boon not activating on gaining minor boon

I purchased the Silver Chalice boon from the merchant, and immediately following in the same window purchased Glowing Orb - with no major boons shown at the buff bar, I closed the merchant screen and received both boons in order without silver chalice activating.

I then re-opened the merchant, and purchased Hare Foot, which caused me to gain hare foot without gaining a Major boon.

I continued on and then at a Pledge room in the next floor, chose a pledge to gain the minor boon "Mirror of fortune" and minor affliction "Costly Aid" - Silver chalice still did not trigger and provide me with a major boon. I still have silver chalice in my list of boons.

This also happened to me at the shop but I was able to trigger the effect by gaining a random boon from a room reward.

After making this bug report - I found a shrine which provides a minor boon, and on activating it successfully triggered Silver Chalice to gain a major boon.

Was that from a restore/boon yellow shrine, or was it from a pledge room reward?

Directly from a normal shrine, not a pledge.
