Door to next level of Sinking Spire tower disappeared

On the first level of the Sinking Spire tower the door interactable to the next level disappeared. I noticed this after I completed the breach on that floor. I saw the door on the minimap prior to completing the breach, after completion it was gone.

Attempted to click around where interactable should be, unable to interact with door.

Went into Atziri Temple and portal out of map to attempt to fix, didn't work.

I am having a similar issue, only I did not notice whether the door was available initially. After clearing the whole floor, there is no door to upper levels. I have tried porting back to hideout and then in again, but still no joy.
EDIT: same map, Sinking Spire

On floor 1, I had a Vaal temple encounter which spawned a boss. I did not get a chest after defeating the boss, either, though maybe that's normal as it did drop loot. The Vaal encounter is marked as complete.
