Blocked from Ancient Vows reward Act 2

While leveling in a party with a guild member in Standard League - I suspect this is related to party play - I was able to pick up the two pieces of the medallion and place them in the alter but there is no option to make a selection and no third charm slot was granted. The quest line shows that the quest had been completed. I have attempted all of the possible bug fixes online such as reloading a new instance, rebooting the game, switching servers, changing various menu and HUD selections. I am on console PS5 and so unable to attempt some of the PC keyboard fixes. It seems that any party play can cause quests to be bricked for players who are not the party leader.
