Cannot complete Corrupted Nexus

I was able to complete the Corrupted Nexuses up to but not including Tier 12.
But the first time I attempted the Tier 12, I died before reaching the boss. When I did reach the boss and killed him, no Beyond Boss was spawned.
So I thought that was fine.

But in the 2nd and 3rd attempts at the Tier 12+ Nexuses, I ran a Tier 12 waystones each time with 4 Mods. I did not die on those attempts, but even after killing the boss PLUS the Beyond Boss that came after, I waited around and no green skill reward item dropped, nor did Doryani appear.

So I am currently stuck at the Tier 12 Nexus level and can no longer progress my Atlas Tree.

I do not know how to reproduce the issue as I do not remember anything other than failing my first attempt by dying before reaching the boss's checkpoint.

Also, I was unable to search this Forum folder (or any other forum folders) with the Forum String search, I tried strings that I saw on the first page like "coin" referring to a thread about the coin not dropping from Yama and there were no search results.
