Hammer of Kamasa does not drop from Act 3 The Molten Vault Boss

+1 not first character on the league, trying to reroll instance, doesn't help.

My first play through of the campaign went very smooth. Now im doing another character and i've ran into 2 bugs of obj items not dropping from bosses, including this one where the hammer doesn't drop after kill.

+1, happened on monk, not on witch

+1 happened on my third char run, now. first two chars did not have this issue, and my 4 chars in 0.1 also did not encounter that issue.

Happened to me just now, 2nd character of the league, killed him three times.

Second character in the 0.3 league non-HC/Ruthless/SSF. I have killed him three times with no luck. Please hotfix ASAP. Thank you
